When something goes wrong on a jobsite, having a complete, timestamped incident record can make the difference between a manageable situation and a serious liability. Raken's Incident Tracking lets your team document incidents in detail right from the field — so your records are accurate, thorough, and ready for review whenever they're needed.

How to Report an Incident on the Web

Step 1: Navigate to Incidents

Log into the Raken web app. Click Projects in the left side navigation and select your project. In the project left side menu, click Safety & QC, then click Incidents.

Step 2: Create a New Incident

Click the orange + Incident button in the top right corner.

Step 3: Select the Incident Type

Choose the incident type that best describes the event — for example, Near Miss, First Aid, Recordable Injury, Property Damage, Equipment Failure, or Environmental. The type determines how the incident is categorized in reports.

Step 4: Fill in the Incident Details

Complete the incident record with as much detail as possible:

  • Date and time — when the incident occurred

  • Location — where on the project it happened

  • People involved — employees or workers involved in the incident

  • Description — a clear narrative of what happened

  • Contributing factors — root cause or contributing conditions

  • Corrective actions — what steps are being taken to prevent recurrence

  • Attachments — photos, files, or supporting documentation

Step 5: Save

Click Save to create the incident record. The record is stored in the project's Incidents section and is available in the incident report export.


How to Report an Incident on Mobile

Step 1: Navigate to Incidents

Open the Raken mobile app and tap Project Tools. Select your project. Tap Safety & QC, then tap Incidents.

Step 2: Create a New Incident

Tap the + button in the bottom right corner.

Step 3: Fill in the Details

Select the incident type, enter the date and time, location, people involved, and description. Attach photos directly from your device's camera or photo library.

Step 4: Save

Tap Save in the top right corner. The incident record syncs to the web app immediately and is available for review by your administrators.


Viewing and Managing Incidents

All incidents for a project are listed in the project's Safety & QC > Incidents section on the web app. Click any incident to open its full record, add follow-up notes, update the corrective action status, or attach additional documentation.

Incident reports can be exported from the project Reports dropdown to share with safety officers, insurance providers, or regulatory agencies.

Can I submit OSHA reports directly from Raken?

Who can see incident records?

Incident records are visible to all team members with access to the project. Administrators can view incidents across all projects from the company level.

Can I edit an incident after saving it?

Yes. Open the incident record and make your changes. All edits are timestamped and the full history is preserved.

Is there a way to track the total number of incident-free days?

Incident counts from Raken feed into your Safety & QC Insights dashboard, where you can track trends over time across your projects.
